“I posted a review previously but want to follow up with specifically what it’s like to be awake during a procedure with Dr. Smith for anyone weighing the general/local anesthesia decision. I’ve done both types of surgery with Dr. Smith and I thought the experience of being awake, not being completely knocked out, was so cool and empowering! I was comfy and relaxed enough to drift off but liked getting to hear Dr. Smith and team in action. Her team is amazing and the way they all collaborate is comforting, fascinating, and distracting from what otherwise could cause some nerves.

The plan: scar revision, removing excess skin to smooth and round me out, and lipo for fat grafting to fill in the divets from my double mastectomy. They kept me feeling perfectly delightful throughout . . . they’d occasionally give me a pill orally (according to a med plan Dr. Smith designed and discussed with me ahead of time) and then shoot water into my mouth with a syringe. Andrea is badass strict about not contaminating her sterile setup.

It wasn’t painful — more like weird, interesting feelings. During numbing, I could feel the initial pokes, but it wasn’t bad. For the rest, it mostly just felt like gentle tugging. Even when they were cutting or stitching, it just felt like dull pressure or pulling. They checked in with me constantly and pretty much always had one person at my head solely for watching after me. They were so calming and comforting - the head massages alone were worth staying awake for. They’d hold my hands, make sure I was warm enough (I was!), adjust blankets, etc. If I was thirsty, needed anything, I knew I could make the smallest noise and get immediate attention. Sometimes we’d chat or I’d lay there comfy and warm and listen to them.

Anyone wondering what happens when you’re awake for surgery and don’t have a catheter? I never wondered until suddenly I thought I HAD to go. Mid-surgery (aka no getting up off the table now) I asked, “Soooo uh what if I really really need to pee like right now?” Everyone was totally unphased: couldn’t even count the bedpans they’d handled. To my great relief, just having the bedpan there in case was all I actually needed. Once it was under me, I didn’t need it.

Despite 7 hours on the table, it was so worth it to do without anesthesia. I felt great afterward and didn’t have all that anesthesia lingering in my body. I didn’t have any actual pain afterwards - more like soreness from a heavy lifting day. Dr. Smith and her team gave me exactly the reconstruction I wanted—what so many others told me wasn’t possible or said I wouldn’t like. But they were wrong. I love it. I feel like myself again—only better. No longer deformed, no longer carrying the trauma of unwanted too-big-implants or the fear of hidden cancer beneath them.

Now, I feel strong. I feel healthy. And most of all, I feel safe. Choosing this surgery, being awake for it, taking control of my health with Dr. Smith by my side—it’s been the biggest, hardest, and most powerful act of self-care I’ve ever done. It wasn’t always easy, but it has absolutely been worth it and the parts with Dr. Smith were easier and better than the parts without her and her phenomenal team.
