“WE needed interim care for a relative post hospital stay, so we began with an initial assessment with Synergy. Our goal was interim care until placement in an ALF. Our needs went from 3-4 days/week to 24/7 and then back to4-5 days/week. Synergy was excellent in providing scheduling to meet our needs. Health care workers were caring, compassionate, and competent. Our relative was sad to leave them behind as he moved on to Assisted Living Facility. Thank you Synergy!!!“

“I've been going to this clinic for 3 years. Dr. Jihan Jacobs and Dr. Louise Mattson are amazing. Compassionate, intelligent and they actually listen! I am working with many autoimmune diseases and these two have helped me orchestrate care among my many specialist. Maggie the nurse is usually avaliable for sick appts but she is still not confident in her people skills. It shows. The support staff are usually very unprofessional, crude and inefficient. . They speak hmong in this clinic.“
