Crossroads Care Center of Sun Prairie

My mom came here for rehab after suffering a fall and stayed for about 2 months. She loved the meals and the staff. As the activated POA, I was very happy with the communication of the staff and the excellent care she received.

This facility provides great quality care to their patients from my experience. Prior to a family friend admitting I looked at nursing home care compare and this is one of the only facilities that is rated 4 stars overall according to CMS in the Madison area. Although the building does look a little older it does not take away from the positive environment and the interactions that the staff build with the patients. I could tell that both the staff and patients were in a good mood when I visited. I asked a nurse what room to go to and the staff member was really friendly and helpful when I asked other questions. My husband was at Crossroads for approx. 2 months for rehab. The RN's, CNA's, and all staff were very friendly and helpful. All seemed to be in good moods. They have activities everyday, have a nice library and lots of puzzles, etc. The people in PT certainly helped my husband get back his strength and he is walking very well. I and my husband would certainly recommend Crossroads. I know most of the staff by first names.
