Hello CEO of the company, The receptionists you have working at 801 is very very rude, I have a eye form that need to be filled out for my business trip so I can get my License renewed they told me 2 2/3 days My trip is this Thursday July 18th 2024 I have to miss my trip to get one form filled out that only take 5 min to fill out eye form ,,,,, Then you charge 25 dollars to fill out a form Iam a patient their who use the services for my eye examine. Other Places don’t charge a fee 25 dollars to fill out an eye form, Once I get my form filled out I will be taking my services somewhere else!!!! I’m glad I read reviews. The way they respond by saying your Medicaid and give an explanation on a public platform instead of calling the customer is crazy. I’ll contact HIPPA to see if it’s a violation. Taking my good federal insurance somewhere else . The location is close to work and home but hard pass

Very unprofessional service and up charged 25$ to authorize a paper. Waited 2 hrs to be seen and the doctor has a very nasty attitude and is racist! They need more employees for better customer service.
