I visited my grandmother today who was recently admitted to Horizon Health. My first experience there was very positive, building is very beautiful and clean. All staff were very welcoming and pleasant. Nurses and CNAs were very professional and helpful. My grandma did not wanted ham in dinner, CNA went and got her alternative from the kitchen. PT team is great and experience.

BE CAREFUL WITH YOUR LOVED ONES. My mother was admitted for rehab purposes only! It was suppose to be 12 day....21 days later she almost died. The lack of care and compassion from the nursing staff. The inadequacy of the nursing staff is beyond understanding. The passing of medications 2 to 3 hours late, not at all, wrong medications given, wrong dose given, medications that were discontinued by her doctor were still offered daily. A nurse actually took a pic of the medication drawers to ask my mom where the missing med was. The lack of education. over use of registry nurses. No continuity of care. She only had the same nurse twice in 21 days. The CNA's are hiding in corners on their phones while your family sits in their own fecal matter. Her bed and room I never saw cleaned once and I was there 3 hours in the morning and a few hours at night each day. A Tums sat on the floor in plain sight for 4 days! If you ask a CNA to come back for hygiene they charted it as refused and you would have to wait until your day again. you only get 2 showers a week. My mom went for 8 days without a shower. She only got her hair washed 1 time in 21 days. Her personal items were just tossed in the closet and when put on isolation, put in bags and left on the floor. She developed bed sores, pneumonia, and pitting edema while in their care. PT/OT maybe once a day or not at all towards the end. The food was unrecognizable not even the staff knew what was being served. Some of the staff that stood out as the worst were CNA Jennifer, she tossed the food tray and water on her bedside table and left, never to return again, LVN Hana messed up her medications, forgot her medications and acted like it was ok. She had an IV order and on her last day was hung and didn't run for 4 hours and it took another hour for the RN to show up and check it. She was abandoned on the toilet multiple times. left in a chair for hours with pitting edema. She was put on isolation and treated like an Aids patient in 1980. Staff did NOT follow C.Diff precautions at all. She was given a strong antibiotic for C.Diff that was an inconclusive results. Promises given to only be empty the next day. Spoke with D.O.N, the floor manager, and manager of PT and again empty promises. The beds are from the 1970's easily and it is lacking in environment, no appeal to the place. When my mother requested to be transported to the ER (within an hour was intubated) they placed all her belonging in one big bag on the floor. I have made several reports to different state agencies and I pray your family has a better experience.

Horizon has done a wonderful job of caring for my 90 year old mother who broke her femur. I see her improving everyday and am grateful for the care she is receiving at Horizon.
