We worked with the team at Connections in Homecare & Communities to help my mom as she neared the end of her life. What started out as just home care 3 days a week quickly turned into 5 days a week and then 24/7 with hospice. Connections in Homecare and Communities connected us to Hospice of the Valley, helped us streamline help with long term care and in the end they gave us the chance to be family not caregivers to my mom. We could not have done it without the team (Denise & Eric Kaye, Allyson Coffin and caregivers: Cori, Cassandra, Astrid, Jennelyn, & Marisa).
Denise and Eric and their entire team were a godsend for us. After my father's unexpected hospitalization. we needed prompt professional and compassionate guidance. The entire staff was responsive in our time of need and, when it came time for dad to be discharged, they found a wonderful and caring facility within our budget and on very short notice. Dad is getting the excellent care he needs, thanks to CHC!
