I use Medforward HIPAA compliant online forms for both my Travel Medicine and Internal Medicine practices. They are easy for both my staff and my patients and travelers to work with. When something needs adjusting, their IT people are very responsive. In 3+ years, I do not recall any issue going unresolved for more than a single business day. I highly recommend them.
