First of all I went to the ER for SOB, palpitations, chest pain.....not the first time ....they show in my chart that someone physically assessed me to and made notes about palpating the abdomen and my skull etc... when in fact they never touched me physically. They gave me a referral for a routine visit at the cardiologist a month from now and I'm struggling to get around from SOB, palpitations, cold sweats, .....the doctor that came into my room stood 4 feet away from me, didn't listen to my heart, he acted like I had some kind of disease and couldn't get close to me. He didn't examine me at all. So tell me.....how can he make any sort of note in his chart or make any...read full review
