Absolutely maddening that you go to a professional place and they tell you to get your phone out, scan the QR code and fill out this extensive pages of detailed information and send it to the directed contact. This is a follow up visit 2 weeks after the last time I was there and TOLD to fill out all the details of my life with dates, medication lists, allergies and Doctors names. 1 time should be enough! Repeat customers should be asked if the information on file is correct or is there anything changed. Several time I have started to just walk out rather than repeat data that should by law retained“
