As other people on this site, I am compelled to tell you this place is a fraud. They billed for covid test kits we never requested or received. I have reported them to Medicare for fraud. DO NOT USE THEM!
Extremely shady place. I've never set a foot in this facility but somehow they had my insurance number, ssn, name and address and submitted a urine analysis to a lab under my information, which resulted in me getting a $552 bill from birdrock lab. I called and was told my PCP that is a partner sent them the wrong patient info and that a manager will send me an email to confirm that my info were wrongfully used. My pcp confirmed they never send them any patient info. I called combined wellness back as I've never received any email and they are avoiding me. It went to the voicemail several times. This is the worst ever experience. A complete scam and fraudulent behavior from the management. A rip off!!!They put on the site a price of 50$ for Rapid Antigen test and though you book a week before your travel, the day of your test they tell you it is 129$ and you are cornered because you have to travel and you have no other options!. Avoid this rip off scam !!!!!!
