It’s been a year since Form officially opened its doors and I am proud to be a client. Dr Leon Waye met me the first time during a consultation and asked me what my goals were and handed me a mirror to tell me everything I loved about myself. That’s how I knew I was in the right place. Dr. Reitmeier is one of the best doctors I’ve ever had the privilege of dealing with. He became ill and had to cancel two appointments, something completely understandable and beyond his control. Doctors are human too.
A couple of cancelled appointments do not erase years of care, dedication, compassion, and professionalism. Dr. Reitmeier will always deserve 5+ stars from me. He deserves understanding and appreciation, not criticism, for having to take care of his own health!
Had 3 PRP shoulder injections here throughout the year and I'm super grateful for the excellent service and very friendly and welcoming staff. Everyone from up front, to the nurses inside, to Dr. Reitmeier were wonderful. Dr. Reitmeier is very knowledgeable and uses the guidance of ultrasound for his PRP procedures, ensuring the injections are of maximal precision. My injury has been finally starting to recover where all traditional methods have failed and I am excited to see the progress within the next 6 months from my final session today. I highly recommend Dr. Reitmeier for those looking into PRP therapy.
