10356 Roosevelt Rd, Westchester“my mother has been coming to Form for about 3 months now and she’s in the best physical therapy care possible. She was previously referred to another physical therapist during most of 2023 and there’s a huge difference in therapists. Her pain has been better managed and she’s been helped a great deal in such little time. Karly and Kevin do an amazing job with patient care as well as making the whole place feel lively and it’s absolutely up to good standards. Anyone who visits them is in safe and trusted hands.“
“My daughter loved her time at Achieve, working alongside Kimberly and Gina! They were so careful and considerate ensuring she got all of her needs met, focused on healing and strengthening after an injury!“
“I was experiencing foot pain from a combination of playing tennis after fracturing my foot and wearing shoes that were applying too much pressure to my foot edge. They helped me regain toe mobility and better balance so that my regular gait is now painless. After reaggravating an old softball finger injury they also taught me some ways to eliminate tightness in my hands. Thank you, Caitlin and team!“
“Dr. Ashlee Kamara is amazing, she definitely knows what she is doing . I found myself a Chiropractic family that cares about my well-being. Now I am on my way to better health after years of low back pain.“

951 W Touhy Ave, Park Ridge“My physical therapy sessions just ended today 4/1/24 and I was SO sad it was my last day! The entire staff was absolutely wonderful and really knew their "stuff". I could tell because I am actually the daughter, sister, sister-in-law and aunt to a family of chiropractors, so I have had experience growing up around people who help restore a person's physical well-being.. This staff was AWESOME, fun to be around, fun to talk to and genuinely concerned with my well being. They made sure that I was comfortable during my sessions too and I really appreciated that. Thank you SO MUCH to everyone there! Jeannine“
“Matt and the staff at this Athletico are wonderful at what they do! They are accommodating, friendly and the physical therapy I received was great! Matt created a routine for my sprained ankle that helped significantly as well as a routine for my kyphosis (back issues) that have changed my quality of life.“
“I received PT from Athletico of Countryside after knee replacement surgery. Most of my hands on therapy was performed the Clinic Manager Michele Alberts. I found her to be extremely knowledgeable in all facets of PT, and more than willing to share that knowledge with you so that you understand both the goals you are trying to reach, and the means being used to achieve them.Michele, her support staff, and all of the other therapists who pinch hit from time to time, are all courteous and helpful. They are also the first ones to tell you that your success is totally dependent upon the amount of work that you put in. I could not have expected more.Ray Heise“
“Dr. Alex Park is amazing! After many visits to the doctor, unable to help alleviate(or identify)my back pain, I finally decided to try out a chiropractor. Lo and behold - Dr. Alex identified the source/cause of my pain and almost immediately put an end to my suffering. The entire staff is very professional and friendly. Highly recommend, also very first time friendly!“
“I had the pleasure of working with Megan at FreeForm, and it truly made a difference. I was referred for PT after experiencing tendinitis in my knee due to a running injury. Megan worked on a rehab plan with me that helped to target and strengthen what was needed, and within six weeks I went from significant discomfort in my knee post-run to running pain free. I really appreciated the individualized care, the attention to detail, and the regular check-ins on my progress. I'm now equipped with a warm-up and strengthening routine I can continue to use into the future. Definitely appreciated everything FreeForm did for me, and would highly recommend.“

2917 W Addison St, Chicago“This is my 2nd experience with sprained ankle that really left me in pain and suffering. Had not heard of PT Solutions before as I had previously did PT at IBJ for initial sprain. I am so so happy I came to PT Solutions and got to work with Hannah! She really is so kind and genuinely wants to help in any problem areas. Love that the exercises were easy to do and I could easily incorporate items at home. That’s for all the encouragement and support Hannah!“

36 Main St Suite 100, Park Ridge“Mandy is a wonderful physical therapist! I have been seeing Mandy on and off for the past 3 years through my 2 pregnancies. One being a c-section and the other a v-bac. She helped me tremendously with my postpartum recovery. From C-section recovery and scar treatment/care to pelvic floor dysfunction and pain. I was also having severe back pain from nursing my 2nd son where I physically felt like I couldn't pick my children up or rock my baby and within a week I was feeling better and within 2-3 weeks the pain was gone and never came back. Mandy also helped me during my 2nd pregnancy prepartum with the aches and pains that came along with pregnancy and caring for a toddler. Along with preparing me with ideas and tips that my husband and sister could use to help alleviate some pain during contractions/labor. As soon as I was able to get back to PT I contacted her immediately. Not only is Mandy a phenomenal PT she is a lovely person. She is friendly, understanding, and easy to talk to. As a mother going through postpartum, it was truly a highlight of my week to spend time with Mandy at our PT sessions for an hour. I highly recommend Mandy to anyone seeking out PT, especially for women's health! Thank you for all you do Mandy!“

“Im in a mobility devuce...a scooter...and the halleats and doorways were way too nsrrow. I had great difficulty maneuverung around and getting into exam rooms. The layout is very handicap unfriendly and there are no power doors. The ekevator eas also a bit difficult because the shape of the elevator had the long side perpendicular to the entry, so i had to turn into it unstead if just pulling firward. Many disabled folk have diffuculty twisting making backing up a blind maneuver. If i could pull in straight and back up straught, that would be much easier that having to back up while turning.“
