Dr Decade is fantastic! Thorough , down to Earth and explains to you so you can understand! I had long wait time to go to a specialist and she went above and beyond to get us in sooner!

The staff here are very friendly and helpful. The office is well maintained and clean also. From check-in to check out, everyone made sure there was nothing needed. They kept us informed of how long the wait would be. I recommend this business for health care.

Pediatrician Joanne Decade is awesome! My son is 14 years old and she explained everything to him, asked questions, gave answers and advices. Very updated on Teenager's concerns. We highly recommend her!

Community Health Centers (CHCFL) is a non-profit organization of Central Florida family health centers that provide high-quality, affordable medical, pediatric, dental and pharmacy care. Community Health Centers Tavares is located in downtown Tavares, just off Lake Dora, and is surrounded by the cities of Eustis and Mount Dora. This Sea Plane themed center provides comprehensive medical, dental, and specialty services, for both children and adults. We offer Family Medicine, Pediatrics, Adult Dental, Pediatric Dental, Lab, and Pharmacy. In addition to preventive care, we also offer an on-site low-cost Pharmacy, exclusive to our patients.
