I had an amazing experience with Dr. Formaker and Ezra. The entire office showed professionalism, compassion, and took time to listen to my concerns. Dr Formaker and his office staff were all lovely. Best cleaning I’ve had in many years, super thorough and done by the dentist himself. All the medical conversations were clearly explained, even down to the how and why we need to floss everyday. Didn’t have all the water and suction tubes in my mouth the whole time which made the visit less stressful. They also had great music playing. So happy my friend recommended. So grateful for the whole experience. Highly recommend for anyone looking for a great dentist.

Dr. Jim has been my dentist since ~1980, when he was a senior student @ USC - the only dentistry practice I could afford at the time! He is a wonderful human being anð a skilled & talented professional. He has seen me through many issues. See the attached "before" and "after" repair pics of my recently broken front tooth. I've referred at least eight family members, friends, and co-workers to his practice. We all happily followed him wherever his office was located throughout the years, though he's been located in Toluca Lake almost all of those years. He specialized in "gentle dentistry" and works caringly with folks who have dental chair anxiety. He is the BEST!
