Failed to address a man’s issue and my concerns during my visit. We are not on the battle field. We are in Santa Maria and an initial visit is more than 2 months long after the scheduled request. The receptionist on duty that day cannot grasp medical terminology nor three syllable words connecting a thought thus making a complete sentence is clueless. The extensive questionnaire is outstanding; however, it had no box to check for those who suffer from high blood pressure and the sheet resembles a mimeograph paper reminding me of grade school back in 1960s.
