“This is an excerpt froma letter I've written to my primary care physician.>>>"For the last five years or so, my feet have been a source of continuing pain. When asked, “How are you doing?” I’d say, “Everything seems to be running alright, except my feet, which often hurt like hell. They have been the first thing(s) to go, which is probably okay, given the alternative. But they still hurt.”Years ago, I read an article or in a book a gerontologist’s explanation for why he started a physical exams with the feet. I’ve tried to find it several times without success, but today I finally located it in Atul Gawande’s Being Mortal (pages 40-42). I’m enclosing a copy (for no reason other than, I can).My wife, Alice, is the lucky person who hears me complain, has asked, “Have you told your doctor about your feet?” I’ve said, “I can’t describe exactly what hurts, or how it hurts or under what circumstances it stops hurting. What’s a doctor to do? It would be like taking a car to a mechanic complaining about something wrong with the engine, but you are unable to describe what the problem is or what the mechanic could do to replicate it and how he’d know that whatever he replicated was what you were complaining about. And it’s my feet, not an eye or something in the chest or in the belly.”Six months ago, Alice, saw Dr. Hai-en Peng, DPM. She had deferred podiatry appointment for years. She felt better after the first visit, so when my whining started up again, she made an appointment for me to see Dr. Peng. My experience was the same as hers. After the first appointment my feet felt at least 90% better. After my second appointment, they felt 98% better. If Dr. Peng’s other patients get this kind of relief, he has quite a practice.Unfortunately, mere competence in providing an established professional service in Santa Barbara is not enough to establish a viable practice in Santa Barbara. If one is starting from scratch, some sort of promotion is probably essential. People who do what’s necessary to acquire the education and training necessary to obtain a professional practice, are probably good at doing a lot of things, but sales and self-promotion might not be one of them.I don’t know if Dr. Peng engages in promotion. I haven’t seen his name on the side of buses, and I have a hard time imaging him making cold calls to potential referral sources. Alice has told me his principal office is in Camarillo and he has set up his Santa Barbara office as a trial to see if there is sufficient demand for his services. I hope he stays here to take care of my feet, Alice’s feet, and for everyone else in Santa Barbara who has sore feet.I know nothing about your referral policy, but the next time you see a sore foot this letter will have been a success if you consider sending your patient to Dr. Peng."“
