The person with the unwell mind in the yellow van blew through an intersection where I had right of way. He had plenty of time to see me coming, and I was pushing a stroller with a baby in it. If I hadn't noticed him speeding up when he saw me rather than slowing down my daughter would be dead right now.

Hired to clean the outside windows in our complex. They missed many windows in the building. When asked whether they cleaned certain windows, they tried saying that they did and gave multiple excuses like the inside was dirty, or humidity was trapped between the panels. After persisting and getting access to their reports, it showed that they indeed had not. Eventually, they said that they would come back to clean them again but they never did and just stopped responding. Many other companies out there that do a much better job.
