Knowingly charged twice for one office visit. We brought my daughter in for a physical, he recommended a flu shot, then charged for the shot (fine) AND an extra office visit. Would not undo the charge when brought directly to his attention. Said it would go to collections if not paid. This happened years ago, but wrong is wrong.
