Black-owned, woman led home health agency providing personal care and skilled nursing services to clients in their homes. Services offered: personal care to assist with bathing, dressing, and home care needs and nursing services for more complex care offering head to toe assessments, medication education and administration of oral, intramuscular, subcutaneous, intravenous medication, wound care assessment and management, community referrals, and maintaining communication and collaboration with community providers for continued referrals and support of their patients. All clients will be offered case management services to coordinate with community resources to address identified needs.
