So moved out of state and in process of moving misplaced dot medical form so thinking that someone with common sense would understand moving is very hectic and things happen they would be kind enough to email it to me. Ohh No they just want more money five dollars to email it. Sad very sad!! Well you got your five dollars and I got this review glad I moved!!!
