Evergreen Pointe Transitional Care

This is the best rehab place around! My mom spent a few weeks there, and I can't say enough great things about the staff and the facility! The nurses went above and beyond to make sure mom was comfortable and happy.
The therapists were patient and caring. The facility was super clean and her room was roomy and comfortable. So thankful
Mom got to stay here and recover!

Evergreen Pointe is an amazing facility over the last 2 years my mom has been there 4 times. They have treated her with the upmost respect and care. The staff is amazing my mom adores them as they do her. They helped to seamlessly transition her over to Oak Hill. They have been a true blessing to my mom and my family during some very difficult times in my moms health journey.

My friend Roger has been in Evergreen for over 2 months now and I am his POA and Roger and I would like to express our deepest sincere Thanks to everyone at Evergreen!! EVERYONE!!!
For being a rehab facility place for seniors, it’s the BEST!!!! Nowadays everywhere is SUCH a disaster!! But NOT at Evergreen!!! It’s a NICE CLEAN place!!! They CARE!!!!! Everyone has been very helpful and outgoing!!! And a special shoutout to the man that drives us to Barnes for Roger’s appointments!!!! He’s the Best!!! Great Driver!!!
