I had a very uncomfortable visit with a provider. I called and voiced my concerns. Another individual contacted me to hear about the visit. I explained what happened. I was told I would not be charged the co-pay for that visit. I received a bill anyway. I emailed Melissa about the bill on 5/23 and was told she would review her notes and get back to me. Nearly a month later nobody has called me back about this. The provider included inaccurate statements in my chart regarding the visit. When I discovered this in early May I contacted Hilda and informed her and requested to have an addendum added to my chart. She stated they had to have a meeting and would get back to me. Nobody called me back about this until I contacted them again nearly a month later. Hilda told me in early June they were going to have a meeting and they'd get back to me. I sent a certified letter formally requesting to have the addendum added to my chart for this visit. Nobody has contacted me to let me know this has been added or that my records are ready for inspection, both of which I'm legally entitled.
I've been receiving care from Dr. Chamberlin for the past few months during my prenatal care. She's lovely for the most part, but my last visit in September left me very distraught. When I told her I was not physically able to receive my pap smear because of pain levels and anxiety, she spent the remaining 15 minutes of my time telling me about another patient that was just diagnosed with stage 4 cervical cancer and how my inaction is irresponsible. Had she been more thoughtful in her care she would have known that I have suffered severe S-Abuse most of my life, making for poking my nether- regions to be both traumatic and painful.
