My Mom is here with a hip fracture and had many comorbid conditions including bad heart, severe anxiety and dementia. The pluses here are the staff is very friendly and smiles a lot. The therapy staff are very good and sweet. The nurses have all been good but there is a little bit of miscommunication and the rooms are very tight/small which I know is the norm for these skilled nursing rehabs. Most of my concerns were addressed in all. Special shout out to Kim, (nurse manager), Sara (restorative nursing assistant), Krys (OT assistant), and Mikayla (speech therapist), Jay and Antoinette CNA’s, and Tauna and Amanda from nursing who always listened and addressed my concerns.... read full review

My 85-year-old mother was diagnosed with a bad UTI five days before Christmas. After four days in the hospital, she was transferred to Citadel Post Acute where she stayed for 15 days. I must honestly express my complete satisfaction with this skilled nursing facility. Everyone treated my mother with dignity and respect, and I sincerely appreciate that. Her physical therapy also helped her come along as she did suffer a few mini strokes due to the high blood pressure associated with the UTI.
In particular I would like to Offer sincere praise to Clariz, the Case Worker. Clariz is an empathetic professional and really knows the ins and outs of her role.... read full review

I was visiting a recently admitted patient. This was my first experience at Citadel. It was at about 1pm and as I walked in, the aroma of lunch smelled delicious. The Staff was pleasant and courteous and everything appeared clean and welcoming. My friend was in bed, and appeared calm and rested. After a few minutes the Dr. Came in to introduce herself and go over patient’s medical history. I left at that point.
